Job summary

Enhabit Home Health & Hospice in Marion, NC is seeking a registered nurse to lead patient care plans in the home setting. You will review patient information, coordinate with branch leadership, and provide training to Licensed Professionals to ensure compliant documentation and high-quality care.

The role requires an active RN license, CPR, at least one year of experience in Medicare home health or hospice, valid driver's license, and strong MS Office skills.

Qualifications

• Graduate of an approved school of professional nursing (RN).
• Valid RN license and CPR certification.
• Minimum of one year experience; Medicare home health or hospice preferred.
• Valid driver's license and automobile liability insurance.
• Basic technology skills including MS Office (Word, Excel).

Responsibilities

• Review and approve patient information submitted by the LP during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to train LPs on documentation to meet regulatory standards and keep staff updated on changes.
• Meet one-on-one with LPs to provide in-depth training when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
