Job summary

Enhabit Home Health & Hospice, one of the nation’s largest home-based care providers, seeks licensed clinicians to join a collaborative team delivering high-quality in-home care. You will review patient information for start of care and re-certifications, educate branch LPs, and ensure documentation meets regulatory standards.

Qualified candidates have nursing or physical therapy credentials, an active state license, CPR, a year of Medicare home health or hospice experience, and strong technical

Qualifications

• Graduate of an approved nursing or physical therapy program.
• Maintain a valid clinical license per state rules.
• At least 1 year of experience in Medicare home health or hospice.
• CPR certification and valid driver's license.
• Experience with Homecare Homebase (HCHB) preferred.

Responsibilities

• Review and approve patient information for start of care, re-certification, resumption of care, or evaluation visit.
• Provide ongoing education and training to branch LPs on documentation requirements to meet regulatory standards.
• Meet one-on-one with LPs for in-depth training when needed.
• Promote quality patient care and maintain relationships with referrals, patients, caregivers, and staff.
