Job summary

Enhabit Home Health & Hospice is seeking a licensed professional (nurse or physical therapist) to review patient information, ensure regulatory compliance, and provide clinical training. The role supports branch leadership and promotes high-quality home health and hospice services.

Candidates should have a valid license, CPR, at least one year of Medicare home health experience, and strong Microsoft Office skills, with in-office work as a norm for coordination.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Must maintain a valid clinical license and CPR certification.
• Minimum of one year of experience.
• Valid state driver license and automobile liability insurance.
• Proficient with Microsoft Office (Word and Excel) and basic technology skills.
• Experience with Medicare home health or hospice settings preferred.

Responsibilities

• Review and approve all patient information submitted by the Licensed Professional during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to provide ongoing education and training to LPs to meet regulatory standards and changes.
• Provide one-on-one training when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers, and employees.
