Job summary

PossAbility, LLC is seeking a Qualified Intellectual Disability Professional (QIDP) to oversee Individual Service Plans (ISPs) for individuals with diverse abilities in Illinois. The role focuses on developing behavior support plans, data-driven reviews, and cross-disciplinary collaboration to ensure compliant, person-centered supports.

You will coordinate ISP and BSP development, conduct regular progress updates, and work with therapists, nurses, and social workers to provide comprehensive

Qualifications

• Bachelor's degree in healthcare, human services, public health, or related field.
• QIDP certified and on the Department’s QIDP database.
• Five+ years of direct care experience for individuals with IDD.
• Ability to support individuals with IDD and develop support strategies.
• Knowledge of home care regulations and best practices.
• Strong leadership and communication skills.
• Excellent organizational and problem-solving abilities.
• Valid driver’s license, proof of insurance.
• Current First Aid and CPR certifications.

Responsibilities

• ISP development, implementation, and monitoring to meet individuals' needs and rights.
• Assist in BSP development and oversight.
• Prepare monthly program reviews with data collection, narrative updates and community involvement.
• Collaborate with therapists, nurses, and social workers for comprehensive support.
• Ensure programs comply with IDHS regulations and other agencies.
• Serve as primary case manager for individuals and ensure continuity across programs.
• Train staff and supervise others where needed.
