Texas Health and Human Services Commission (HHSC) is seeking a QIDP III at Mexia State Supported Living Center to oversee the ISP process for individuals on the caseload and lead IDT coordination. The role emphasizes coordination, documentation, and ensuring timely follow-up on recommended supports in a complex care setting.

The position requires a bachelor’s degree in a human services field and at least one year of direct experience with intellectual or developmental disabilities.

Responsabilités

• Oversees the ISP process for all individuals on the assigned caseload.
• Facilitates and documents IDT meetings (ISP Preparation, ISP Addendum, Annual ISP).
• Ensures assessments and evaluations determine functioning, preferences, needs and recommendations.
• Coordinates with the individual and LAR with staff in delivering needed supports.
