Job summary

Qualifications

• Bachelor’s degree in a related human services field is required.
• At least one year of experience working or volunteering with persons with intellectual/developmental disabilities.

Responsibilities

• Oversee ISP process for all individuals on the caseload and facilitate IDT meetings.
• Ensure assessments and evaluations are aligned with IDT determinations.
• Coordinate participation of individuals and LARs with staff to develop needed supports.
• Monitor ISP implementation and adjust plans as needed.
• Train and coach staff on service delivery and behavior management.
