Qualifications

• Bachelor’s degree in psychology, sociology, social work, special education, rehabilitation counseling, or related field.
• At least one year of experience working directly with persons with intellectual or developmental disabilities.
• Strong knowledge of IDT processes and ISP development.

Responsibilities

• Orchestrates and oversees ISP process for all individuals on the caseload.
• Facilitates IDT meetings and ISP preparations and addenda.
• Coordinates and follows up on assessments, plans and supports.
• Ensures timely documentation and compliance with ICF/IID standards.

Education

Bachelor's degree in psychology, sociology, social work, special education, rehabilitation counseling, or related human services field
