Qualifications

• Bachelor’s degree in psychology, sociology, social work, or related human services field.
• At least 1 year experience working or volunteering with persons with intellectual or developmental disabilities.
• Pass fingerprint background check and related screenings.

Responsibilities

• Orchestrates and oversees the ISP process for all individuals on the caseload.
• Facilitates IDT meetings and annual ISP.
• Ensures assessments and evaluations are conducted as identified by the IDT.
• Coordinates participation of the individual and LAR with staff to develop needed supports.
• Ensures timely follow-up for recommended services and equipment.
