Job summary

Texas Health and Human Services Commission seeks a Qualified Intellectual Disability Professional II to oversee ISP processes and coordinate the IDT for individuals in the state center. You will lead multidisciplinary teams, document plans, and ensure compliance with ICF/IID standards.

The role requires a bachelor’s degree in a human services field and at least one year of relevant experience, with strong organizational and communication skills.

Qualifications

• Bachelor’s degree in psychology, sociology, social work or related human services field.
• At least one year experience with intellectual/developmental disabilities.
• Strong organizational and communication skills for ISP and IDT processes.

Responsibilities

• Oversee ISP process for all individuals on the caseload.
• Lead and coordinate interdisciplinary team (IDT) meetings.
• Monitor and adjust active treatment plans as needed.
• Coordinate with LARs and paraprofessional staff to deliver supports.
• Ensure timely follow-up and documentation in electronic records.
