Job summary

Health & Human Services Comm (HHSC) in Lubbock State Supported Living Center is seeking a Qualified Intellectual Disability Professional II to oversee ISP processes and lead the Interdisciplinary Team (IDT). This role emphasizes individualized planning and coordination of supports for residents with intellectual and developmental disabilities.

The QIDP II will document plans, monitor progress, and ensure timely follow-up for complex needs.

Qualifications

• Bachelor’s degree in a related human services field required.
• Minimum of 1 year experience with intellectual/developmental disabilities.
• Experience coordinating ISP and IDT processes preferred.
• Ability to work with individuals with complex behavioral needs.

Responsibilities

• Oversee ISP process for all individuals on caseload.
• Facilitate and document IDT meetings throughout the year.
• Monitor assessments to determine functioning, preferences, strengths, and needs.
• Coordinate with individuals and Legally Authorized Representatives for supports.
• Document and follow up on recommendations for services, equipment, or programs.
• Train staff in service delivery and behavior management.
