Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant to join our in-house Skilled Nursing Facility partner in Temple, OK. This full-time position supports resident care under the supervision of a Physical Therapist.

You will conduct screenings, assist with evaluations, help develop and implement treatment plans, document in NetHealth EMR, and educate residents, families, and staff.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying target goals and treatment plans.
• Conduct individual and group treatment including strengthening, mobility, gait, and modalities.
• Assist in selecting techniques/media and adapting as indicated by resident performance.
