Job summary

Evolve Therapy Services, LLC seeks a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in Temple, OK. You will work under the supervision of a Physical Therapist to deliver therapy services and support care plans.

Responsibilities include screenings, treatment planning, and delivering individual and group sessions, documenting in NetHealth EMR, and collaborating with the rehab team to achieve resident goals.

Qualifications

• Active Physical Therapy Assistant license in state(s) of practice.
• Graduate of an accredited PTA program with an associate degree.
• Commitment to ongoing continuing education and licensure maintenance.
• Ability to work under the supervision of a Physical Therapist.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under supervision.
• Assist in identifying goals and treatment plans.
• Deliver individual and group treatments focused on strength, mobility, balance, and gait.
• Assist in selecting techniques/media and adapt activities as needed.
• Monitor residents’ response and communicate with the Physical Therapist.
• Assist in planning and modifying resident schedules with the team.
• Ensure timely documentation and compliance with CMS in EMR systems.
