Job summary

Evolve Therapy Services in Oklahoma City, OK is seeking a Physical Therapy Assistant to support an in-house Skilled Nursing Facility. You will work under a Physical Therapist to provide therapy services and implement treatment plans.

The role emphasizes screenings, goal setting, individual and group treatment, accurate documentation in NetHealth EMR, and ongoing professional development. Requires an associate degree and active PTA license.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist in establishing short- and long-term goals and treatment plans.
• Conduct individual and group treatment as outlined in the treatment plan.
• Assist in selecting techniques/media and adapt as indicated by resident performance.
• Monitor resident response to intervention and consult with the Physical Therapist for modifications.
• Establish and modify resident schedules in coordination with the Physical Therapist and team.
• Adhere to department and resident schedule; modify as needed for treatment regimen.
• Train resident, family and staff in equipment use and application.
• Participate in resident, family, and staff education.
• Maintain sound judgment in evaluation and follow-up of therapy programs.
• Report to work on time and maintain high productivity during shifts.
• Participate in Quality Assurance activities as required.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.

Education

Associate's degree in physical therapy from an accredited program

Active Physical Therapy Assistant license in state(s) of practice
