Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in Temple, OK. This full-time role supports the Physical Therapist in delivering resident-specific therapy programs and ensures timely documentation in NetHealth EMR.

Responsibilities include screenings, treatment implementation, progress tracking, resident and family education, and collaboration with an interdisciplinary team to maintain high-quality resident care and CMS

Qualifications

• Associate's degree in physical therapy required.
• Active Physical Therapy Assistant license in state(s) of practice required.
• Ongoing continuing education and licensure maintenance required.

Responsibilities

• Provide PT services to residents under the supervision of a Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist PT in evaluations and in treatment planning.
• Contribute to resident schedules and documentation in NetHealth EMR.
• Educate residents, families, and staff on therapy programs.
• Maintain quality resident care and CMS compliance.
