Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Temple, OK. You will deliver PT services under supervision, assist in screenings, and contribute to treatment plans to improve resident mobility and function.

You will document care in NetHealth EMR, participate in patient education, and coordinate with the Physical Therapist and care team to optimize outcomes. The role supports growth and ongoing professional development.

Qualifications

• Associate degree in physical therapy from an accredited program is required.
• Active Physical Therapy Assistant license in state(s) of practice is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and treatment planning under the Physical Therapist.
• Contribute to treatment plans with goals and modalities.
• Document CMS-compliant care in NetHealth EMR.

Education
