Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Temple, OK. The PA will support the Physical Therapist with resident care, screenings, treatment planning, and both individual and group therapy activities to improve mobility and function.

The role requires an accredited PTA program, an active license, and ongoing licensure maintenance, with CMS-compliant documentation in NetHealth EMR. Full-time position with competitive benefits.

Formación

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsabilidades

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention and treatment.
• Assist in evaluating resident progress and treatment planning with the Physical Therapist.
• Help establish resident schedules and coordinate with the care team.
• Deliver individual and group therapy activities including strength, mobility, gait, and ROM.
• Assist in selecting techniques/media and adapt as indicated by resident performance.
• Document compliance and outcomes in CMS-relevant systems (NetHealth EMR).
• Educate residents and families; participate in QA activities as required.
