Job summary

Evolve Therapy Services is hiring compassionate therapists for our partner in Oklahoma City, OK. This position is for a Physical Therapy Assistant in an in-house skilled nursing facility, with full time, part time, or PRN options.

You will support the Physical Therapist, conduct screenings, and implement treatment plans designed to improve residents' mobility and function while maintaining CMS-compliant documentation.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluation and treatment planning with the Physical Therapist.
• Develop short-term and long-term goals and treatment plans with the PT.
• Deliver individual and group treatment addressing strength, mobility, balance, and gait.
• Assist in selecting techniques/media and adapt as needed with PT input.
• Monitor residents’ response and modify as indicated.
• Maintain resident schedules with the team and adhere to CMS documentation requirements.
• Train residents, families, and appropriate staff in equipment use.
• Participate in education for residents and staff and contribute to quality assurance.
