Evolve Therapy Services in Saint Paul, MN is seeking a compassionate Physical Therapy Assistant to join our in-house Skilled Nursing Facility team on a PRN basis. You will work under the supervision of a licensed Physical Therapist to deliver resident-focused therapy and help maintain CMS-compliant documentation.

The role emphasizes evaluations, treatment planning, patient education, and collaboration with residents, families, and staff to promote mobility, strength, and independence.

Qualifications

• Associate's degree required.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsabilités

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Contribute to treatment planning and goals.
• Conduct screenings and assist in evaluations as directed by the Physical Therapist.
• Support in scheduling and coordinating treatments with other team members.
• Document compliant and medically necessary CMS documentation in NetHealth EMR.
