Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Sutherland, NE. This PRN role supports the supervising Physical Therapist in delivering patient-centered therapy and contributes to resident mobility and quality of life.

Responsibilities include screenings, assisting in evaluations, developing goals and treatment plans, documenting in NetHealth EMR per CMS standards, and educating residents, families, and staff.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying targets and goals for treatment plans.
• Conduct individual and group treatments as outlined in the treatment plan focusing on mobility, strength, balance, and ROM.
• Maintain documentation in NetHealth EMR per CMS expectations.

Education

Associate's degree in physical therapy

Active PTA license in state(s) of practice
