Job summary

Evolve Therapy Services in West Saint Paul, MN is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility. The role is full-time or PRN, supporting the Therapy team under the supervision of a Physical Therapist.

Responsibilities include conducting screenings, assisting with evaluations and treatment plans, and delivering both individual and group therapy sessions. Emphasis on documentation and resident/family education within CMS guidelines.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention.
• Assist in evaluations and developing treatment plans with PT guidance.
• Develop target goals and treatment plans with PT input.
• Deliver individual and group treatments including mobility, strength, gait, and ROM activities.
• Train residents, families, and staff on equipment use.
