Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Le Center, MN. The role is PRN, providing therapy under a Physical Therapist's supervision to residents in a nursing facility setting.

Responsibilities include screenings, contributing to evaluations, and implementing treatment plans with a focus on mobility, strength, gait, and functional activities. Documentation and interdisciplinary collaboration are essential parts of the role.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsabilités

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and in developing a treatment plan with goals and modalities.
• Conduct individual and group therapy sessions as outlined in the treatment plan.
• Assist in selecting techniques/media and adapt as indicated by resident performance.
