Job summary

Evolve Therapy Services, LLC is seeking a Physical Therapy Assistant (PRN) for an in-house Skilled Nursing Facility in the Minneapolis area. You will work under the supervision of a Physical Therapist to deliver and coordinate patient-centered therapy services.

Responsibilities include screenings, treatment planning, individual and group therapy, documentation in NetHealth EMR, and collaboration with the care team. A state PTA license and ongoing education are required.

Qualifications

• Must hold an active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education to maintain licensure.
• Assumes responsibility for professional development to maintain credentials.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention or treatment.
• Assist the PT in evaluations and in treatment planning with goals.
• Conduct individual and group treatments per the treatment plan.
• Monitor resident response and modify as directed by the PT.
• Assist in scheduling and coordinating treatment with the PT and team.
• Document in NetHealth EMR systems per CMS expectations.
• Participate in resident, family, and staff education.
