Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant to join our in-house SNF team in West Saint Paul, MN. You will work under the supervision of a Physical Therapist to deliver individualized therapy plans and help residents improve mobility and functional independence.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in the evaluation and development of treatment plans under the Physical Therapist.
• Assist in establishing and modifying resident schedules in conjunction with the Physical Therapist and team.
