Job summary

Evolve Therapy Services in Saint Paul, MN is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility. The role supports the Physical Therapist in delivering patient-centered care and implements treatment plans with supervision.

Responsibilities include screenings, assisting with evaluations and goals, conducting individual and group interventions, training residents and families, documenting in NetHealth EMR, and participating in quality assurance activities.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluations under the direction of the Physical Therapist and identify goals and treatment plans.
• Assist in selecting and sequencing PT techniques/media as indicated by resident performance.
• Monitor residents' response to intervention and modify as needed in consultation with the PT.
• Assist in establishing resident schedules with the PT and care team.
• Educate residents, families and staff on equipment use and interventions.
• Complete compliant CMS documentation in NetHealth EMR and participate in QA activities.

Education
