Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Shakopee, MN. The PRN position supports resident care under a Physical Therapist's supervision and contributes to ongoing therapy goals in a team-based setting.

Responsibilities include screenings, treatment delivery, progress documentation, and collaborating with the PT and care staff to optimize resident outcomes.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist in identifying target goals and treatment plans.
• Conduct individual and group treatment per treatment plan including strength, mobility, gait, ROM, and modalities.
• Assist in selecting techniques/media and sequence of activities per resident performance and PT guidance.
• Monitor residents' response to intervention and consult with PT for modifications.
• Assist in scheduling residents with PT and other team members.
• Adhere to resident schedules and modify as needed for treatment regimens.
• Train residents, family, and staff in equipment use and safety.
• Participate in resident, family, and staff education.
• Ensure compliant documentation in CMS expectations via NetHealth EMR.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license in state(s) of practice
