Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Minneapolis, MN. This PRN role supports resident therapy under the supervising Physical Therapist.

Qualifications

• Must hold an active Physical Therapy Assistant license in state(s) of practice.
• Continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Screen residents to determine need for intervention and treatment.
• Assist in evaluation, goal setting, and treatment planning with the PT.
• Deliver individual and group treatments including strength, mobility, gait, and ROM activities.
• Document CMS-compliant notes in NetHealth EMR.
• Educate residents, families and staff on therapy plans and equipment use.
• Coordinate resident schedules with the PT and care team.
