Job summary

Continuum Therapy Partners, LLC in Chicago, IL is seeking a Part-time Physical Therapist to support our Norwood Crossing location. We value clinician voice and collaborative care to improve patient outcomes.

The role requires a Doctor of Physical Therapy (DPT), a current IL PT license, NPTE, and the ability to develop and adjust treatment plans with patients/families; prior PT experience is preferred. Generous PTO and 401k with company match are available.

• Current, unencumbered IL PT license; NPTE required.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• ?Doctor of Physical Therapy (DPT) degree required


• ?Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)


• ?1 year of previous experience as a Physical Therapist preferred
