Job summary

Continuum Therapy Partners in Lancaster, WI is seeking a Physical Therapist to join our daytime, full-time team. You will evaluate, develop, and implement individualized treatment plans in coordination with other disciplines, and educate patients and families on home programs.

Applicants must hold a Doctor of Physical Therapy (DPT), a current, unencumbered state license and NPTE, and while one year of PT experience is preferred, we value therapists who share our commitment to patient-centered

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
