Job summary

Continuum Therapy Partners in Chicago is seeking a licensed Physical Therapist to join our team at Norwood Crossing. A Doctor of Physical Therapy (DPT) degree is required, with a current state license and NPTE eligibility. This is a part-time daytime position.

You will evaluate, develop, and implement individualized treatment plans in collaboration with other disciplines, document patient progress, and educate patients and families on home exercise programs and injury prevention.

Qualifications

• Must hold a Doctor of Physical Therapy (DPT) degree.
• Current, unencumbered state Physical Therapy License; NPTE must be passed.
• 1 year of experience as a Physical Therapist preferred.

• Documents patient progress and adjusts plans of care based on clinical outcomes

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
