Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist at Sauk County Health Care Center in Reedsburg, WI. The role requires a Doctor of Physical Therapy (DPT) and an active Wisconsin license, with NPTE completed. A year of PT experience is preferred, with a focus on evidence-based plans of care and patient education.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
