Job summary

Continuum Therapy Partners in Chicago, IL is seeking a licensed Physical Therapist to evaluate, treat, and coordinate care for patients in a flexible PRN schedule at Norwood Crossing. The ideal candidate holds a Doctor of Physical Therapy (DPT) and an active state license, and collaborates with a multidisciplinary team to deliver evidence-based care.

This role emphasizes patient education, individualized home exercise programs, and outcome-driven plans, with opportunities to impact recovery

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; NPTE required.
• 1 year of experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
