Job summary

Continuum Therapy Partners in Wisconsin is seeking a licensed Physical Therapist to join our Sauk County Health Care Center team in Reedsburg. This role emphasizes patient-centered care and evidence-based practice to improve outcomes.

You will evaluate and develop individualized treatment plans, document progress, educate patients and families on home programs, and collaborate with other disciplines to optimize recovery.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
