Job summary

Qualifications

• Bachelor's degree in social work, psychology, or related field.
• Strong communication and interpersonal skills; ability to protect confidentiality.

Responsibilities

• Conduct household visits and center-based interviews to assess needs.
• Develop and implement action plans with clients and families.
• Discuss options and involve clients in decision making.
• Assess risk and provide psychosocial support.
• Link clients to services and community resources.
• Maintain complete documentation and monitor service provision.
• Provide psychosocial counseling and coordinate services with other providers.
