Consumer Direct Care Network in Austin, TX is seeking a Service/Program Coordinator to manage a Medicaid client caseload and coordinate in‑home care needs in alignment with federal/state regulations and MCO rules. You will enroll clients in services and conduct regular check‑ins to update care plans as needs change.

The role emphasizes effective written and verbal communication, collaboration with stakeholders, and proactive problem‑solving.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology or related field preferred.
• 1 year experience with individuals with disabilities or elderly preferred.
• Combination of education and experience.
• Employees must provide reliable internet for remote/hybrid work.

Responsibilities

• Maintain and coordinate a caseload of clients enrolled in Medicaid programs
• Coordinate clients’ in-home care needs per Federal/State regulations, MCO rules, and contract requirements
• Assist with daily intake and referral processes to support business growth
• Promote cohesiveness in the office environment
• Support day-to-day operations and client advocacy
• Collaborate with stakeholders regarding service delivery
• Provide excellent customer service to internal and external clients
• Enroll clients and employees in services, web portal, and EVV systems as needed
