Job summary

Consumer Direct Care Network is seeking a Support Broker/Consultant to coordinate day-to-day implementation of contractual requirements within Medicaid guidelines across the state. You will train, enroll, report, audit, and ensure contract deliverables are met, while working with members on plan and budget development, submission, and revision.

This role requires a four-year degree in Human Services or a related field and relevant experience; travel to client homes is expected.

Qualifications

• Requires four-year degree in Human Services or related field and one year experience with elderly or disability populations.
• Alternative: four-year degree in any subject with two years experience in the target populations.
• Or two-year degree in Human Services with five years experience in the target populations.

Responsibilities

• Apply person-centered approach to keep client at the center of program functions.
• Coordinate day-to-day contractual requirements within Medicaid guidelines.
• Create care plans, train clients on services, audit services, and report findings.
