Job summary

Consumer Direct Care Network seeks a Service Coordinator Supervisor to oversee intake, referrals, and in-home care plans for Medicaid clients. You will supervise a caseload, ensure regulatory compliance, and guide staff to deliver high-quality care across our network.

You will coordinate with providers and stakeholders, lead professional development, and support program growth while advancing patient advocacy nationwide.

Qualifications

• Bachelor’s degree preferred in Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology or related field.
• 3 years experience working with individuals with disabilities or the elderly preferred.
• Combination of education and experience acceptable.
• Leadership experience preferred.
• Ability to work flexible and/or extended hours, if needed, to meet job requirements.

Responsibilities

• Supervise and maintain a caseload of clients enrolled in Medicaid programs.
• Supervise clients’ in-home care needs in compliance with regulations and contract requirements.
• Supervise daily intake and referral processes for positive business growth.
• Provide leadership and training to other staff.
• Promote office cohesiveness and client advocacy.
• Lead special projects including large-scale enrollments and transitions.
