Job summary

Consumer Direct Care Network in Wisconsin is seeking a Service Coordinator to help manage Medicaid client caseloads and coordinate in-home care services.

You will ensure compliance with state and federal regulations, communicate effectively with clients and providers, and support program growth through proactive intake and referrals. This role requires a driving position and strong organizational skills.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology, or related field preferred.
• 1 year of experience working with individuals with disabilities or the elderly preferred.
• A driving position; valid driver's license required. Internet access for remote/hybrid work if applicable.

Responsibilities

• Maintain and coordinate a caseload of clients enrolled in Medicaid programs.
• Coordinate in-home care needs in compliance with Federal/State regulations, MCO rules, and contract requirements.
• Assist with daily intake and referrals to support growth.
• Promote office cohesiveness and provide client advocacy.
• Represent the company at stakeholder meetings and health fairs.
• Communicate effectively with clients, families, and providers.
