Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapy Assistant to support in-house skilled nursing facility services in Warren, PA. This PRN role focuses on assisting the Physical Therapist, conducting screenings, and implementing treatment plans to improve residents' mobility and function.

Responsibilities include individual and group sessions, documenting progress in NetHealth EMR, and collaborating with the care team to meet CMS standards.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state of practice.
• Commitment to ongoing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluations and develop target goals and treatment plans with the Physical Therapist.
• Conduct individual and group treatments focusing on strength, mobility, balance, and ROM.
• Document responses and progress, and adapt treatments as indicated.
