Job summary

Evolve Therapy Services, LLC is seeking a licensed Physical Therapy Assistant to support an in-house Skilled Nursing Facility in Altoona, PA. The PTA will work under the supervision of a Physical Therapist to deliver high-quality resident care.

Responsibilities include screenings, evaluations assistance, treatment planning, individual and group therapy, and documentation in NetHealth EMR. You will educate residents and staff while adhering to CMS guidelines and facility schedules.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluation and treatment planning under the Physical Therapist's direction.
• Implement treatment plans including strength, mobility, balance, and gait activities.
• Document care in NetHealth EMR per CMS requirements.
• Educate residents, family and staff on therapy plans and safety.
• Maintain schedules and ensure timely, quality care.
