Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Familiarity with CMS documentation and NetHealth EMR is a plus.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention or treatment.
• Assist the Physical Therapist in evaluation and goal setting.
• Develop and modify treatment plans with the Physical Therapist.
• Deliver individual and group therapies focusing on strength, mobility, balance, and gait.
• Assist in selecting techniques and adapt activities per resident progress.
• Monitor resident responses and modify plans as directed by the Physical Therapist.
• Support creation and maintenance of resident schedules with the therapy team.
• Assist with and participate in Quality Assurance and CMS-compliant documentation.
