Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in Bradford, PA. This full-time, part-time, or PRN position supports the Physical Therapist in delivering care and contributing to treatment plans for residents.

The PTA will conduct screenings, assist in evaluations, and provide individual and group therapy sessions, documenting progress in NetHealth EMR while maintaining compliance with CMS standards.

Qualifications

• Minimum requirement: Associate's degree in physical therapy.
• Must hold an active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide PT services under supervision of Physical Therapist.
• Conduct screenings to determine intervention need.
• Assist in evaluation and treatment planning with PT guidance.
• Support in creating target goals and treatment plans.
• Perform individual and group treatments per plan, including mobility and gait training.
• Assist in selecting techniques/media and adapt as needed.
• Document compliant and medically necessary care in NetHealth EMR.
