Job summary

Evolve Therapy Services is hiring a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Clarion, PA. The PRN position supports the Physical Therapist and the care team to deliver resident-centered therapy services.

You will assist in screenings, evaluations, and treatment plans, while coordinating schedules and educating residents and families on therapy activities and equipment. CMS-compliant documentation is required.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and treatment plan development under the Physical Therapist.
• Assist in establishing and modifying resident schedules with the team.
• Train residents and families in use of equipment and participate in education.
• Maintain compliant documentation in NetHealth EMR per CMS expectations.
