Job summary

Evolve Therapy Services in Altoona, PA is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility. This full-time or PRN position supports the Physical Therapist in delivering resident care and executing treatment plans.

Responsibilities include screenings, evaluations assistance, individual and group treatments, documentation in NetHealth EMR, and education for residents, families, and staff.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Ability to work under supervision of a Physical Therapist and follow treatment plans.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluations and treatment plan development under the Physical Therapist.
• Deliver individual and group treatments focusing on strength, mobility, gait, ROM, and modalities.
• Document compliant and medically necessary CMS documentation in NetHealth EMR.
