Job summary

Evolve Therapy Services, LLC is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Hastings, PA. This PRN position supports the Physical Therapist in delivering therapy services and ensuring quality resident care.

Responsibilities include screenings, treatment under supervision, and documentation in NetHealth EMR. The ideal candidate holds an active PTA license and an Associate's degree, with commitment to ongoing education and professional development.

Qualifications

• Associate's degree in physical therapy is required.
• Active Physical Therapy Assistant license in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist the Physical Therapist in identifying goals and treatment plans.
• Deliver individual and group treatment per the plan, addressing strength, mobility, balance, and ROM.
• Document compliant and medically necessary notes in NetHealth EMR.
• Coordinate resident schedules with the Physical Therapist and team.
• Educate residents, families, and staff on therapy techniques and safety.
