Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Altoona, PA. This role provides therapy services under the supervision of a Physical Therapist, conducts screenings, contributes to evaluations, and supports treatment planning and execution to improve resident outcomes.

The position involves conducting individual and group treatments, maintaining CMS-compliant NetHealth documentation, and collaborating with the care team

Qualifications

• Must have an Associate's degree in physical therapy.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations under the direction of the Physical Therapist.
• Assist in treatment planning with short-term and long-term goals.
• Conduct individual and group treatment as outlined in the treatment plan.
• Monitor residents’ response to intervention and modify as needed.
• Assist in scheduling in conjunction with the Physical Therapist.
• Train residents, family and staff in equipment use.
• Participate in resident, family and staff education.
• Maintain CMS-compliant documentation in NetHealth EMR.
