Job summary

Amedisys, part of the Optum family, is hiring a Per Diem Physical Therapist to deliver skilled home health services. You will promote mobility, restore function, and improve safety in patients’ homes while collaborating with caregivers and a care team.

You will participate in 30-day reexaminations, discharge planning, and adjust interventions as needed. A comprehensive benefits package is offered, with pay comprising per-visit points and hourly rate.

Qualifications

• Active physical therapy license in the state of assignment.
• Graduate from CAPTE-accredited PT program.
• Minimum 1 year of PT home health experience; less than 1 year requires regional approval.
• Current CPR certification.
• Driver's license and reliable transportation.

Responsibilities

• Provide skilled PT services in patients' homes to improve mobility and safety.
• Conduct reexaminations at minimum every 30 days and modify plans as needed.
• Perform discharge planning and coordinate care with the interdisciplinary team.
• Other duties as assigned by supervisor.
