Job summary

Amedisys, part of the Optum family, is hiring per diem Physical Therapists to deliver skilled PT services in patients' homes. You will promote mobility, restore function, and support independence as part of an interdisciplinary care team.

Responsibilities include patient movement, reexaminations, discharge planning, and collaboration with caregivers. Requires CAPTE-accredited program, state license, CPR, and 1+ year home health PT experience; drivers license required.

Qualifications

• Graduate from CAPTE-accredited physical therapy program.
• Current license to practice physical therapy in the state of assignment.
• Current CPR certification.
• 1+ year of related physical therapy experience for home health care.
• If less than one year of experience, regional clinical leaders' approval and mentoring/residency participation may be required.
• Driver’s License and reliable transportation.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning.
• Other duties as assigned.
