Qualifications

• Current license to practice physical therapy in assigned state.
• Graduate from CAPTE-approved PT program.
• One year PT experience in home health preferred.
• CPR certification current.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions and conduct reexaminations (every 30 days minimum).
• Modify interventions as needed and perform discharge planning.
• Other duties as assigned.
