UnitedHealth Group, in collaboration with Amedisys, is seeking a Per Diem Physical Therapist to deliver skilled PT services in patients' homes, promoting mobility, function, and safety. You will work with patients, families, and care teams to support recovery and independence.

The role offers per diem scheduling, competitive pay, and the flexibility to manage visits across assigned regions, with emphasis on quality care and adherence to licensing and safety standards.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• 1+ year of related physical therapy experience for home health; mentoring if less than a year may be required.
• Current CPR certification.
• Driver's License and access to reliable transportation.

Responsabilités

• Provide skilled physical therapy services to patients in their homes to promote mobility and safety.
• Perform reexaminations at minimum every 30 days and discharge planning as needed.
• Collaborate with patients, families and interdisciplinary care team; participate in care planning.
