Job summary

Amedisys, part of the Optum family, is seeking a Per Diem Physical Therapist to deliver skilled PT services in patients' homes. You will promote mobility, restore function, and improve safety and independence while collaborating with caregivers and the care team.

Responsibilities include initial and periodic reexaminations, discharge planning, and adapting interventions. You will join a supportive environment with benefits and opportunities for professional growth.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• 1+ year of related physical therapy experience in home health care.
• If less than one year, regional clinical leaders' approval and mentoring/residency participation may be required.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days (at minimum), modify interventions as necessary, and perform discharge planning.
• Participate in a care team with patients and caregivers to support recovery and independence.
• Other duties as assigned.
